We are now using our built-in Elation medical record scheduling system instead of IntakeQ.
However, this appointment request form will also remain active if you are having trouble finding something online that works. This will be most useful for phone visits or vaccine-only visits. We can usually find a spot in the schedule manually that won’t show up online.
